Let's say that everyone uses the correct torque when tightening hardware for the sake of this discussion. Indexmarks also called witness mark is usable until something gives in the construction/assembly of parts. If someone is re-applying torque overtime and the witness marks no longer match something is giving in the construction. If your torque wrench says xx nm but the witness mark moved passed its original spot, something has changed since the last time you torqued the part down. First and most common. It can be threads, screws that stretch pass it's spring back point and it starts to deform with permanent damage. It has reached its Yield point. Or it's the material that's being clamped down that moves away or get worn down because of movement or bad clamp force to begin with. Third option is that the clamp itself is giving and starts to give after repeated use over time. To reach the same Clamp force again re torque is needed. But Work hardening starts to take shape, fatigue is the initiation and propagation of cracks in a material due to cyclic loading. Only way to solve that is to beef up the construction/part where strength is needed to prevent stretch, movement and spring back. Usually, the only part one wants to stretch or act like a spring in an assembly is the Screw since it's a part that's easy to change and cheapest to change.
